Certification

Applicant attests that the accompanying sales and distribution reports are complete and accurate, that the release date recorded is the date of the first shipment from the manufactures to the distribution network, that the figure for units shipped is net of all recordings returned to date and that any returns are not anticipated to be sufficiently large in number to disqualify this application.
